What Does a DOT Physical Include?

A FMCSA DOT physical exam follows a standardized protocol designed to ensure commercial drivers meet the federal medical standards required to safely operate a commercial motor vehicle (CMV). Here's what to expect:

Medical History Review

A comprehensive review of your health history including past surgeries, chronic conditions, medications, and any conditions that may affect your ability to safely operate a commercial vehicle.

Vision Testing

Distant visual acuity must be at least 20/40 (Snellen) in each eye with or without correction. Peripheral vision, color recognition (traffic signal red/green/amber), and ability to recognize vehicles and road markings are evaluated.

Hearing Evaluation

You must perceive a forced whispered voice in the better ear at not less than 5 feet, with or without the use of a hearing aid. A formal audiometric test may be performed as needed.

Blood Pressure & Pulse

Blood pressure and pulse rate are measured and documented. Drivers with high blood pressure may receive a time-limited certificate requiring follow-up evaluation per FMCSA guidelines.

Physical Examination

A head-to-toe physical exam assessing the musculoskeletal system, neurological function, cardiovascular system, respiratory system, abdomen, and extremities for any conditions that could impair safe driving.

Urinalysis

A urine sample is collected and tested for signs of underlying medical conditions such as diabetes (glucose), kidney disease (protein), and other abnormalities. This is a dipstick test, NOT a drug screen.

Cardiovascular Assessment

Evaluation of heart sounds, rhythm, and any history of cardiac conditions including coronary artery disease, heart failure, or arrhythmias that may affect your CDL medical certification.

Neurological & Mental Health Screening

Assessment for any neurological disorders, epilepsy, loss of consciousness, or mental health conditions — including medications — that could impact the safe operation of a commercial vehicle.

Note: The DOT physical urinalysis is NOT a drug test. Substance abuse testing (DOT drug & alcohol) is a separate program administered by your employer's designated employer representative (DER) — not part of the medical exam.

Walk-In Hours & Location

No appointment needed for DOT physicals. Walk in during any of our open clinic hours and our FMCSA-certified medical examiner will complete your exam. Most visits take less than 60 minutes and you'll receive your certificate before you leave.
